PERSONAL SUPER MEMBER FORMS

Application form

Member application is in the Product Disclosure Statement
- apply to become a member of Aon Master Trust Personal Super.

Contribution forms

Super contributions
- make lump sum or regular contributions by cheque.

Request to transfer whole balance of super benefits between funds
(includes Aon Master Trust compliance letter)
- roll in/transfer full super benefits from another fund.

Direct debit request and agreement
- arrange for regular contributions to come out of your bank account.

Spouse contributions splitting from the Aon Master Trust
- split your contributions with a spouse.

Spouse contributions splitting into the Aon Master Trust
- split your contributions in another fund and transfer them to your spouse’s account in the Aon Master Trust.

General forms

Change member details and options
- provide or change member details
- apply for or change death and/or TPD insurance (there are also separate forms for insurance purposes)
- indicate or change investment option
- nominate or update dependants/ legal representative (non-binding nomination)
- provide your tax file number.

Binding death benefit nomination
- to make a binding nomination on your account.

Renewal of binding death benefit nomination
- to renew a current binding nomination on your account

Revocation of binding death benefit nomination
- to revoke an existing binding nomination on your account

Superannuation benefit transfer or payment request
- request a transfer/rollover of your benefits to another super fund
- request a benefit payment.

Tax file number notification
- provide your tax file number to the trustee.

Member information request
- request information from Aon Superannuation Pty Limited where it is acting as trustee of your benefits.

Authority to advise
- appoint a personal financial adviser
- authorise the payment of adviser service fees.

Notice of intent to claim a tax deduction
Self-employed members can:
- claim a tax deduction
- reduce a previous claim for a tax deduction.

Switching investment options - current members
- to change investment options for current account balances and future cashflow.

Insurance forms

Death, TPD and income protection – short form
- apply for or increase death and/or TPD cover up to $800,000 and/or income protection up to $8,000 monthly.

Death-only insurance cover – short form
- apply for or increase death-only cover up to $1m.

Personal statement and declaration of health
- apply for or increase your insurance cover.

Insurance questionnaire – reclassify your occupation
- update your occupational status for insurance purposes.

Insurance update
- change insurance options or opt out of insurance.

Continuation of income protection insurance
- continue income protection cover on transfer from Corporate Super to Personal Super.

Authority to disclose personal and sensitive information
- give approval for authorised parties to share your personal information to process an insurance application.

Individual insurance transfer questionnaire – for new member
- to transfer your current insurance cover with a superannuation fund or retail insurer to the Aon Master Trust.

Individual insurance transfer questionnaire – for existing member
- to transfer your current insurance cover with a superannuation fund or retail insurer to the Aon Master Trust.

Letters

Compliance letter
- provide, to any fund that requests it, evidence that the Aon Master Trust is a complying fund under the Superannuation Industry (Supervision) Act.

QROPS registration letter
- provide, to your UK fund on request when transferring a UK pension, evidence that the Aon Master Trust is a QROPS fund.
