Contribution forms
General forms
  • Change member details and options
    - provide or change member details
    - apply for, change or opt out of death and/orTPD (there are also separate forms for insurance purposes)
    - apply for, change or opt out of income protection insurance cover
    - indicate or change investment option
    - nominate or update dependants/ legal representative (non-binding nomination)
    - provide your tax file number.
  • Member information request
    - request information from Aon Superannuation Pty Limited where it is acting as trustee of your benefits.
  • Authority to advise
    - appoint a personal financial adviser
    - authorise the payment of adviser service fees.
Insurance forms
  • Death, TPD and income protection – short form
    - apply for or increase death and/or TPD cover up to $1,000,000 and/or income protection up to $8,000 monthly.
Letters
  • Compliance letter
    - provide, to any fund that requests it, evidence that the Aon Master Trust is a complying fund under the   Superannuation Industry (Supervision) Act.
  • QROPS registration letter
    - provide, to your UK fund on request when transferring a UK pension, evidence that the Aon Master Trust is a QROPS fund.
