• Spouse contributions splitting from the Aon Master Trust
  - split your contributions with a spouse.

• Spouse contributions splitting into the Aon Master Trust
  - split your contributions in another fund and transfer them to your spouse's account in the Aon Master Trust.

• Change member details and options
  - provide or change your details
  - apply for, change or opt out of death and/orTPD (there are also separate forms for insurance purposes)
  - apply for, change or opt out of income protection insurance cover
  - indicate or change investment options
  - nominate or update dependants/ legal representative (non-binding nomination)
  - provide your tax file number.

• Internal transfer within the Aon Master Trust
  - to request a transfer within Aon Master Trust superannuation products.

• Binding death benefit nomination
  - to make a binding nomination on your account.

• Renewal of binding death benefit nomination
  - to renew a current binding nomination on your account

• Revocation of binding death benefit nomination
  - to revoke an existing binding nomination on your account

• Tax file number notification
  - provide your tax file number to the trustee.

• Member information request
  - request information from Aon Superannuation Pty Limited where it is acting as trustee of your benefits.

• Authority to advise
  - appoint a personal financial adviser
  - authorise the payment of adviser service fees.

• Notice of intent to claim a tax deduction
  Self-employed members can:
  - claim a tax deduction
  - reduce a previous claim for a tax deduction.

• Switching investment options
  - to change investment options for current account balances and future cashflow.

• ATO Authority to provide your tax file number to your super fund
  - to authorise your employer to provide your tax file number (TFN) to your super fund.
  

• Death, TPD and income protection – short form
  - apply for or increase death and/or TPD cover up to $1,000,000 and/or income protection up to $8,000 monthly.

• Personal statement and declaration of health
  - apply for or increase your insurance cover.

• Insurance – reclassify your occupation
  - update your occupational status for insurance purposes.

• Life stages cover
  - apply for life stages cover for death and TPD.

• Insurance update
  - change insurance options or opt out of insurance.

• Authority to disclose personal and sensitive information
  - give approval for authorised parties to share your personal information to process an insurance application.

• Individual insurance transfer questionnaire
  - to transfer your current insurance cover with a superannuation fund or retail insurer to the Aon Master Trust.

• Compliance letter
  - provide, to any fund that requests it, evidence that the Aon Master Trust is a complying fund under the   Superannuation Industry (Supervision) Act.
